Full Time: $300 – Includes room, meals, instruction, and T-shirt Check In: Fri. June 22nd Noon
Part Time: $220 – Includes, meals, instructions, and T-shirt Check Out: Sun. June 24th Noon

PARTICIPATION REQUIRMENT:
Campers must supply a copy of a current physical or doctors release to participate in rigorous physical activity. Please let the staff know of any medications you are taking.

I give permission for my child to be treated when necessary by Flagstaff Medical Center for injuries or illness. It is also understood that no major surgery will be performed on my child without my further specific consent, except in the case of extreme emergency when the delay in obtaining such consent would constitute a serious risk of life to my child. I further realize that expenses for treatment beyond those normally covered by camp insurance shall be my responsibility.
